Transcript Requests

If you have any questions, you can email the office at chhsoffice@musd.org.

To request an unofficial ($5) or official copy (signed/sealed for $7) of your transcript, please see the options below:

1. Mailed Request - You can request a transcript through the mail.  Please mail your request to:

In the envelope, please include:

- $7/$5 in exact change or money order made payable to Milpitas Unified School District

- a copy of your ID. Please do not send your actual ID.

- a forwarding address (where to send the transcript) physical or email

- your phone number (just in case there is an issue with the transcript)

- full name during attendance and last year attended
